JOB TITLE: Case Manager – Trafficked Youth
PROGRAM: Survivor Support Services
SUPERVISOR: Coordinator, Human Trafficking Program
WORKING HOURS: 40 hours per week
QUALIFICATIONS:
Education: AA plus 4-5 years’ experience or BA/BS plus 2-3 years’ experience in the social work or human services area required. MSW strongly preferred.
Experience:
- Experience in providing direct human services is required.
- Experience serving victims of violence and/or trauma is required.
- Experience in the area of cultural competency is preferred.

JOB SUMMARY:
The primary role of the Case Manager involves providing intensive, comprehensive, and culturally competent services to minor survivors of human trafficking and those that are at-risk of being trafficked within the Western New York Community. Direct services include victim advocacy, crisis intervention, safety planning, general intake, service assessments, and coordination of support services both on-site and throughout Erie County. The Case Manager will work with the Coordinator of Human Trafficking Services to strengthen strong community partnerships with youth-specific service organizations in order to ensure a quality, community response to trafficked youth. This relationship building may include information/education during youth support groups and/or for the general community, as needed. The Case Manager will also be asked to assist in identifying impediments to safety and independence for trafficked youth identified in Erie County. On-call responsibilities occur periodically.
DUTIES AND RESPONSIBILITIES:
- Conduct intake with clients, including service needs assessments, developing culturally and linguistically appropriate service plans, safety assessments, and safety planning;
- Assist in stabilization of clients upon escape/recovery, which may include coordination of services with law enforcement and victim advocacy;
- Assess client’s emergency, short-term and long-term needs through intake process;
- Provide intensive case management including referrals for off-site services including, but not limited to shelter, mental health support, health screenings and medical care, substance use treatment, court accompaniment, employment support, and public benefits acquisition;
- Ensure that all survivors understand their rights as victims of crime and facilitate application to associated status and benefits including NYS OVS applications, NYS OTDA referrals, and federal certification and/or eligibility;
- Participate in all individual and group supervision meetings;
- Attend relevant trainings and/or conferences related to subject matter expertise;
- Adhere to agency documentation and data collections standards and procedures, including data submission for all grant requirements;
- Adhere to organization client confidentiality policies and procedures;
- Complete tasks as needed regarding program evaluation;
- Perform other duties as required.
WORKING CONDITIONS:
- Must be able to travel to sites using own personal vehicle and must follow all state and local auto safety requirements such as seat belt and child car seat use.
- Schedule may periodically include weekend and/or evening hours as needed to meet the demands of our clients
